As an NHS-funded programme, we are obligated to obtain certain information from everyone referred to our programme. Therefore, in order to begin the Diabetes Prevention Programme, you will need to complete two NHS Initial Assessment Surveys first.
After creating your account in the Liva app, you will see a prompt on the “Today” page to complete the first survey called, "Your daily activities". If you close it, don't worry; you can find it again by navigating to the top of the “Today” page. Simply click on it to fill it out.
Once you have completed this one, the next survey called, "Complete your initial assessment" will be unlocked.
This process typically takes no more than 15 to 20 minutes. If you cannot complete it in one session, you can save your progress and return to it later.
Once you've completed both surveys, you'll be prompted to fill in your start height and weight followed by being able to schedule a video consultation with a health coach.
