What is the Living Well Taking Control Programme?

The aim of the programme is to help you improve your knowledge, ability and confidence whilst offering you the support you need to make good lifestyle choices

What topics will the programme cover?

  • Know your health and understand what Type 2 diabetes is
  • Healthy lifestyle and behaviour change
  • Healthy eating
  • Understand food labelling
  • Create an individual physical activity plan
  • Learn how to plan ahead
  • Set individual weight management goals
  • Learn about the effects of alcohol

Can I bring someone along to the group sessions with me?

Yes, in fact it is encouraged! If you have a family member or carer that would like to attend with you, please let LWTC know.

I have a disability / I don’t speak English - will my needs be catered for?

Yes, you can discuss any additional needs with LWTC directly.

How do I join the face-to-face group programme?

If you have already selected the digital programme with Liva, please contact our support team on help@livahealth.com and ask them to arrange for you to transfer over to the face-to-face programme with LWTC. This will require your discharge from our programme. We will then request that LWTC arrange your transfer, and they will arrange contact with you directly to book you into a local group.

If you have not yet registered for the digital programme, you can contact LWTC directly on info@lwtcsupport.co.uk and they will help you with booking you onto a local group.
