The programme is 9 months long and aims to help you to make lifestyle changes to reduce the risk of developing diabetes in the future. The idea is that these changes are small and realistic for your own individual circumstances, the most important thing about them being that they must be sustainable for you, and your health coach will support this.

The programme will start with a live initial video call with your chosen health coach who will work with you to get to know you a little better, and establish goals with you to reach your long-term aims from joining the programme. They will then send you personalised video and written messages to keep you motivated, support you and provide helpful tools and resources via our Liva app. This will be weekly for the first 12 weeks and then fortnightly for the remaining 6 months of the programme.

You will learn a range of information including nutrition, physical activity, stress, weight management if desired, and how to make changes that you can stick with long term. This information is sent as part of a curriculum that is scheduled to be sent to you on an automated basis each week. The information is designed to be suitable to everyone so you may disregard what is not relevant to you and your health coach can then support you with applying any relevant topics to your own priorities and goals during your next check-in.

Your health coach is your partner in progress, but they do need your input to guide you best. Activity shows them what's working, what needs adjustments, and how to tailor your programme best for you.

The more you track goals, habits, and metrics, the clearer your roadmap to success becomes. Regular updates to your coach will help you to identify patterns, celebrate wins, and stay motivated.

Please note that you will require a smartphone or tablet device to access the programme.
